Scheduling Information & Registration Forms

Consult appointments are scheduled in the afternoons Monday through Thursday. Surgeries are scheduled in the morning and through mid-day Monday through Friday. If you are in pain or have an emergency situation, every attempt will be made to get you scheduled as soon as possible.

We do our best to stay on schedule in order to minimize your wait time. Due to the fact that we provide surgical services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.

 

Patient Registration

You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

When preparing for your first visit to the office please first download and review this document.  This is very important if you are unaware of HIPPA regulations. 

The Office for Civil Rights enforces the HIPAA Privacy Rule, which protects the privacy of individually identifiable health information; the HIPAA Security Rule, which sets national standards for the security of electronic protected health information; the HIPAA Breach Notification Rule, which requires covered entities and business associates to provide notification following a breach of unsecured protected health information; and the confidentiality provisions of the Patient Safety Rule, which protect identifiable information being used to analyze patient safety events and improve patient safety.
